THE STUDENT MUST LIVE IN PUTNAM COUNTY TO ATTEND PUTNAM COUNTY SCHOOLS

Students must be 5 years old by August 15^th to register for Kindergarten.

IN ORDER FOR YOUR CHILD TO REGISTER AT BURKS ELEMENTARY SCHOOL THE FOLLOWING IS NEEDED FOR ALL NEW STUDENTS:

1. PROOF OF RESIDENCY

A. (1 of the following) Property Tax Receipt for previous year, rent receipt, lease from landlord, or payment of insurance of property

B. (1 of the following) Current bill, WIC, Bank Statement, check stub

NO LICENSE OR PERSONAL LETTERS WILL BE ACCEPTED

ADDRESS WILL BE SUBJECT TO VERIFICATION

2. BIRTH CERTIFICATE (a certified copy with an official number and seal) NO MOTHER’S COPY

3. SOCIAL SECURITY CARD (copy)

4. TENNESSEE SHOT RECORD (from a health care provider, NO BLUE CARD)

5. PHYSICAL (within the year of the school start date, from health care provider)

6. PROOF OF CUSTODY (if the student is not living with one or both parents)

7. PARENT/GUARDIAN ID
